Death certificates in Oklahoma are held at the state level, not at the county level.

To order a death certificate contact the Oklahoma State Department of Health Division of Vital Records.

Steps to order by mail.
    1. Download application
    2. Complete application in full.
    3. Enclose photocopy of photo ID.
    4. Enclose correct fees.

Certificates can also be obtained at the

Vital Stats Office

McAlester, OK 74501

Documentation of how you are blood related to the person whose certificate you are requesting is required.
